1. Have you or anyone in your household had any of the following symptoms in the last 14 days: sore throat, cough, chills, body aches for unknown reasons, shortness of breath for unknown reasons, loss of smell, loss of taste, or a fever of 100.0 degrees or greater.(Required)
2. Have you completed or started your COVID Vaccine injections?(Required)
3. Do you plan to get the COVID Vaccine when eligible?(Required)
4. Given the current pandemic do you understand that there are inherent risks associated with being present at First Baptist Church of James Island and agree to assume all risks associated with it?(Required)

 First Baptist Church of James Island is adhering to all federal and state guidelines regarding COVID 19. Social distancing will be practiced at all times. Masks are required upon entrance. The church will be sanitized before and after each service. All dates may be subject to change.
